2. Click here to print a form to submit a check made payable to Robert Packer Hospital Auxiliary, then you can either:

• Mail to Robert Packer Hospital Auxiliary, C/O Guthrie Development, One Guthrie Square, Sayre, PA 18840.
• Drop off your form and payment at The Connections Gift Shop in the main lobby of Robert Packer Hospital located at One Guthrie Square, Sayre, PA 18840.
